As our doctors all have appointments at local hospitals, we are ideally placed also for ‘Rapid Review’ clinics – providing immediate post discharge follow-up for patients who have recently been sent home from hospital. We aim to re-visit medication changes, improve communication of diagnoses and management plans to patients and their families, hence working together with their GP to better manage chronic conditions and reduce re-admission rates to hospital. We also have a large network of sub-specialty physicians that can provide timely access to review, should it be required.
